Medical In-Home Habilitation

 

Medical In-Home Habilitation at Unity Developmental Disabilities Care Center (UDDCC) provides individualized, medically-focused skill-building services for individuals with developmental disabilities or traumatic brain injuries who require additional health-related supports in order to safely live at home. This service is designed for people with more complex medical needs who still benefit from habilitation, independence training, and person-centered support in their home environment.

Medical In-Home Habilitation combines daily living skill development with health-related support to ensure each individual can participate in meaningful activities while maintaining safety, stability, and well-being.

Medical In-Home Habilitation may include:


Daily Living Skills


  • Dressing, hygiene, grooming
     
  • Meal preparation and safe feeding techniques
     
  • Household cleaning and organization
     
  • Laundry and personal belongings care
     
  • Community access skill-building
     

Medical-Related Support (non-nursing tasks)


  • Assistance with prescribed positioning, mobility, and range of motion
     
  • Monitoring of health conditions and documenting observations
     
  • Following RN-developed care plans
     
  • Assistance with medical equipment such as:
     
    • walkers
       
    • gait belts
       
    • communication devices
       
    • adaptive equipment
       

(NOTE: All medical tasks requiring a nurse are completed by or overseen by a licensed nurse per state regulations.)


Behavioral & Emotional Support


  • Calming strategies
     
  • Routine structure and sensory support
     
  • Trauma-informed care practices
     

Skill-Building & Independence Training


  • Money management basics
     
  • Communication and social skills
     
  • Safety awareness and problem-solving
     
  • Self-advocacy and personal decision-making
     

Care Oversight & Nursing Support


UDDCC ensures every individual receiving Medical In-Home Habilitation has:

  • An RN-developed support plan and medical protocols
     
  • Regular RN oversight based on health needs
     
  • Trained habilitation staff who understand:
     
    • health monitoring
       
    • documentation
       
    • emergency procedures
       
    • appropriate responses to changes in condition
