Youth Continuous Home Services

 

Youth Continuous Home Services at Unity Developmental Disabilities Care Center (UDDCC) provide a stable, long-term, family-style living environment for children and youth who cannot safely remain in their family home. This service is designed for young people who need consistent support, structure, and supervision due to foster care placement, developmental disabilities, behavioral needs, or trauma history.

Our Youth Continuous Homes operate 24/7 and are staffed by trained caregivers who create a nurturing, predictable, and therapeutic environment where youth can heal, grow, and work toward permanency goals.

UDDCC Youth Continuous Home Services include:

  • Safe, stable, and supervised living environment
     
  • Trauma-informed care and behavioral support
     
  • Life skills training: hygiene, chores, communication, cooking, budgeting
     
  • Educational support, school coordination, and tutoring as needed
     
  • Medication assistance and health monitoring by trained staff
     
  • 24/7 supervision and crisis support
     
  • Transportation to school, appointments, and activities
     
  • Family visitation coordination and reunification support (when appropriate)
     
  • Case management, ISP support, and goal setting
     
  • Community participation and social development
     

Our highly trained staff receive:

  • Mandt training
     
  • CPR/First Aid
     
  • Abuse & neglect prevention
     
  • Trauma-informed care
     
  • Medication training (per protocol)
     
  • Documentation & ISP goal tracking
     
  • Crisis intervention and supervision training
